Massive Cochrane Review Finds Vaping Beats Nicotine Patches for Quitting Smoking, With Key Caveats

A major new systematic review published in the Cochrane Database of Systematic Reviews finds that nicotine e-cigarettes are more effective than traditional nicotine replacement therapies (NRT) such as patches and gum for helping smokers quit. Drawing on data from 80 randomized controlled trials involving nearly 30,000 smokers, the review provides high-certainty evidence that approximately four additional smokers per 100 will successfully quit using nicotine e-cigarettes compared with NRT, and about two additional quitters per 100 compared with non-nicotine e-cigarettes. Published as a "living systematic review" that will be continuously updated, it represents one of the most comprehensive assessments of smoking cessation methods currently available.
Lead author Nicola Lindson of the University of Oxford emphasized the importance of offering smokers multiple options, while co-author Jamie Hartmann-Boyce of the University of Massachusetts Amherst stressed that the findings apply only to regulated nicotine e-cigarettes, not unlicensed or disposable devices, which may pose safety risks such as unstable batteries and harmful chemicals. Hartmann-Boyce described the evidence for regulated products as "very strong."
The review carries significant limitations. It does not examine whether those who quit smoking via vaping subsequently quit vaping itself, nor does it resolve the long-term health risks of vaping, given the relatively short history of these products. Co-author Chris Bullen of the University of Auckland acknowledged these gaps as "genuine concerns that require separate evidence reviews." No serious short-term harms were detected, but the authors called for larger and longer-duration trials.
The review comes against the backdrop of tobacco use killing more than 7 million people globally each year. Researchers argued that expanding the range of proven cessation options, including regulated nicotine vaping, could meaningfully reduce tobacco-related mortality, even as questions about vaping's long-term effects remain under active investigation. Funding came from Cancer Research UK, the National Cancer Institute, and the FDA Center for Tobacco Products.
